WE ARE H.E.R.

Health Equities RPM, LLC (H.E.R.), a minority-woman-veteran-owned telehealth consulting company who, with our team of health equity partners, have developed a model, the H.E.R. Community-Based Health Equity Model built on our “Four Pillars”, to address health care disparities that impact certain urban and rural population groups at the beginning of life and along the continuum of care. To accomplish its mission, H.E.R. has partnered with Global Telehealth Services, CareOrbit, Me.Health, BodiMetrics, and others to establish the Health Equity Partners.

To leverage technology and culturally competent programming to address health care disparities that impact the lives of people of color and other socially and economically disadvantaged groups.

OUR VISION

That on a day soon to come, everyone, no matter the color of their skin or their socio-economic status, will have fair and equal access to respectful care needed to be as healthy as possible.

THE PROBLEM

Profound disparities by race and ethnicity, as well as social class, exist not only in outcomes that include life and death, but also in underlying conditions—such as poor access to affordable and nutritious food, decent, safe and sanitary housing and unhealthy neighborhood environments—that undermine health.

Health Inequities Cost Everyone!

New research funded by the National Institute on Minority Health and Health Disparities, part of the National Institutes of Health, revealed that racial and ethnic health disparities, across five racial and ethnic minority groups nationally and for all 50 states and the District of Columbia, using a health equity approach, cost the U.S. economy $451 billion as of 2018. More recently Deloitte has found similar economic impact.​ (Source)

In our comprehensive care approach, we prioritize collaboration with key services that specifically target and address the Social Determinants of Health, ensuring holistic support for our community’s well-being.
