The Health Equities RPM Team is committed to bringing care into the homes of the underserved and especially people of color, to address health and social inequities highlighted by the COVID-19 pandemic. Community-based strategies supported by RPM will better enable older adults and other high utilizers of acute care services with complex social and health care needs to live as independently as they can for as long as they can at home. Remote patient monitoring also can help eliminate “maternity care deserts” that impact the quality of maternal care for new and expectant mothers of low and moderate income by giving them 24/7 access to culturally competent care, education and supportive services needed to manage their health and that of their newborns. For public agencies such as public health departments, public housing authorities and community action agencies concerned with health care inequities that impact their constituents, RPM improves access to the care they need and want. For health care systems and public and private payors, RPM may help address the challenge of rising healthcare costs. In short, RPM holds benefits for everyone with a stake in improving health outcomes for underserved urban and rural population groups, and especially people of color. The Health Equities RPM Team is laser-focused on playing a role in this future.
