To further its mission, H.E.R. is teaming with Global Telehealth Services (“GTS”), based in Hudson, Ohio. The GTS Remote Patient Monitoring SaaS platform, GTS VirtualHealth, empowers health care teams to monitor and analyze streamed vital data from remotely located patients. The platform, the most secure of its nature, also provides for video conferencing using HIPPA compliant video. GTS VirtualHealth operates on IOS and Android phones and tablets and works with most Bluetooth, USB, Wi-Fi or cloud-based medical devices and integrates with most EMR platforms.
H.E.R. has teamed with Care Orbit, LLC to create the H.E.R Maternal Care App with Care Orbit Education and Engagement, a “one-stop shop” that provides education and engagement, remote monitoring (via a link to GTS), and traditional synchronous tele-health capabilities.
Upon opening the App on a smartphone or tablet expectant mothers will enter their personalized maternal Care Orbit putting at their fingertips links to the resources they need for healthy pregnancy outcomes. The App with Care Orbit education, engagement and links to supportive services that address social determinants of health, creates a deeply-engaging HIPAA-compliant learning environment for expectant mothers by applying best practices from the engagement industry to create proprietary Storytelling Blueprints that map all key content to directly drive targeted health literacy and positive actions important to maternal and infant health outcomes.
The Care Orbit can be accessed on any screen (smartphone, tablet or desktop) and does not require special training or downloads to track individual usage to help ensure active engagement with key information. Nor does it require alteration of existing EMR or office systems or processes. With the Care Orbit guide comes a series of motivational/educational outbound messages to help bring key information into focus at key points throughout each trimester and a baby’s first year. The simple-to-use, very patient-centric nature of the Care Orbit guide has been clinically proven to drive significant positive results.
A two-year study tracking the effects of Care Orbit on patient knowledge, complications and accountability showed that the guide increased health literacy by +68%, decreased re-admissions by 41%, increased satisfaction by 22%, reduced office calls due to forgotten information by over 50% and showed improvement in compliance in 9 in 10 patients. All usage of the guide is tracked in order to ensure its effectiveness and help the Community Care Connect team identify information areas of greatest interest and need.
The BodiMetrics’ Prevention Circul+ Ring will accurately measure a range of metrics that are integral to health, including continuous blood oxygen (Sp02), heart rate, finger temperature and sleep stages. The Ring also is expertly designed to collect HRV (heart rate variability), blood pressure and a single lead ECG. The Ring is fully integrated into GTS Virtual Health. Patient data will be sent to the patient’s GTS dashboard monitored 24/7 by culturally competent healthcare professionals. Data can also be sent to patients’ smartphones, transmitted to a secure server, and shared with caregivers. The Ring is the only wearable that takes into account pigmented skin, a significant competitive advantage.

Community Care Connection, Inc. (“CCC”), a 501c3 organization established by H.E.R. to provide third party community-based remote patient monitoring services on behalf of H.E.R.’s clients, is the leading edge of H.E.R.’s health equity initiatives. Childbearing women participating in the H.E.R. Maternal Health Equity Initiative will be issued a H.E.R Community Care Connection Kit to include a Wi-Fi enabled BP cuff, a heart and SPO2 monitoring device, a Wi-Fi enabled weight scale, and other wearables as needed, which may include glucose meters. Each CCC nurse-monitor will support a panel of pregnant women, monitored daily, 24/7/365.
Each day CCC nurse monitors will log into the H.E.R./GTS portal to monitor mothers’ vital signs and review and respond to surveys designed for specific pregnancy-related issues. If a would-be mother’s vitals fall outside pre-configured parameters, a trigger response protocol is initiated, facilitating triage and real-time intervention. A CCC nurse monitor may intervene via video, secure message through the App, or coordinate an in-person visit by a community health worker/doula. CCC’s culturally competent nurse monitors will be prompted to call on the expectant mother and ask if she is experiencing abdominal pain, headache, changes in vision, swelling of hands or face, nausea or vomiting, shortness of breath, and anxiety. If abnormal readings persist care providers will be notified by the nurse-monitor on duty at the time. For mothers who elect not to be monitored by the CCC team, data from their readings can be transferred directly to their medical record for their preferred providers to review and speed provider intervention when needed.
Via the App, participants also have 24/7 access to supportive services that address social determinants of health, and perinatal and postpartum education on topics such as delivery, breastfeeding, good nutrition and more to promote healthy pregnancies and birth outcomes.

The Birthright Doula Collaborative was established by H.E.R. to train and develop community health workers to become Community Care Connection Digital Doulas providing digital support via the H.E.R Maternal Care App for H.E.R’s clients and the birthing women they serve.
Virtual doula support on a digital health platform is associated with lower odds of caesarean birth and an improved birth experience. Experience also shows that digital health support childbearing can play a meaningful role in increasing Health Equity and birth outcomes.
CCC digital doulas will provide emotional and social support for childbearing women and their families, which includes non-clinical support during pregnancy, labor, and birth, as well as assistance during the transition to parenthood in the initial postpartum period. Such support also includes education and logistical planning to help reduce stress associated with preterm labor.
Doula support during pregnancy and childbirth is associated with improvements in outcomes including lower rates of cesarean section and preterm birth, higher rates of breastfeeding initiation, and increased satisfaction with the birthing experience. Access to doula care helps to address inequities in pregnancy outcomes for black women and women in high poverty neighborhoods.
In addition to improved outcomes, evidence suggests that continuous, one-to-one doula support can lower total medical expenditure by reducing the likelihood of complications and increasing the likelihood of a healthy mother and baby.
The Collaborative is led by a team of experienced community health worker/doulas who have trained hundreds of lay people across several states to become certified doulas.
